Update Voter Information

Address Change:  (Voter's address changes within the State of Florida)
May be changed by completing a Florida Voter Registration Application Form or by phone. Verification of your Florida Driver's License Number/Florida ID issued by the DMV, or the last four (4) digits of your social security number; whichever is on file with the Supervisor of Elections, will be needed

Name Change:
Must be changed by completing a Florida Voter Registration Application Form (English) [PDF] (Español) [PDF]. 

Party Change: 
Must be changed by completing a Florida Voter Registration Application Form (English) [PDF] (Español) [PDF]. 

Date of Birth Correction: 
Must be changed by written correspondence or by completing a Florida Voter Registration Application Form (English) [PDF] (Español) [PDF].  

  • Written correspondence must include the voter’s signature and either the date of birth or voter registration number.

Signature Update: 
It is very important to update your signature on file, as it may change throughout the years. Signatures on your registration record are used to verify signatures on petitions, vote-by-mail ballots and provisional ballots. If your signatures do not match, your petition or ballot will not count.

Replacement Voter Information Card: 
Must be requested by completing a Florida Voter Registration Application Form (English) [PDF] (Español) [PDF].

Voter Registration Cancellation: 
Must be cancelled by a written request from the voter. Written correspondence must include the voter’s signature and either the date of birth or voter registration number.

To Report/Remove a Deceased Voter: 
A copy of a death certificate issued by a governmental agency authorized to issue death certificates must be submitted to the Supervisor of Election's office.


Vote-By-Mail Ballot Request: 
A vote-by-mail ballot may be requested by the voter or with the voter’s permission, by a member of the voter’s immediate family or the voter’s legal guardian.  It may be requested by phone, email, on the department’s web site or by written request. The request must have the voter’s date of birth, residence address, and the voter's Florida Drivers License Number/Florida ID Number or the last four (4) digits of their social security number.  Requests made by someone other than the voter must include their name, address, relationship to the voter, and if available, their drivers license number.  Written requests must also include the voter’s/requestor’s signature. 

  • Ballots mailed to an address other than one on the voter’s record must be submitted by signed written notice.  Persons in the military or residing overseas are exempt from this requirement. 
  • Address changes may be made from a vote-by-mail ballot request if made by the voter.
